How to Enroll 

1. Print and complete fully the Corporate Application.
2. Print and complete fully the Employee Enrollment Form for each person.
3. Print and complete fully the Payment Authorization Agreement
4. Attach a cheque payable to SmartChoice C.I. Benefits in the amount equal to your 1^st months premium. 

Example:      Total Volume of Insurance divided by 1000 X $1.024 X 8% PST = Premium

                        Eg. 10 employees electing $25,000 coverage each 

                        Total Volume of Insurance - 10 X 25,000 = $250,000 divided by 1000 

                        Monthly premium calculation is 250 X $1.024 X 8% = $276.48 

Please include a cheque marked VOID for banking purposes.
